CountryHouse in Folsom

Our Folsom community is the newest CountryHouse. Opened in June 2021, this beautiful and modern residence features both private and companion studios and has space for 50 residents to make CountryHouse their home.

CountryHouse in Folsom is located on Iron Point Road, just down the road from Kaiser Permanente and near El Dorado Freeway. Though this community is brand new, a sister residence is located nearby Granite Bay. Families in the area have trusted CountryHouse to care for their loved ones since 2015.

Our Leaders

The heart of CountryHouse really shines through in our people. Our team members understand the difference between caring and giving care, and they’re committed to infusing our philosophy of treating people like family into everything they do.

Jimmy Duong joined CountryHouse as Executive Director in November 2022. He brings a variety of experience to his role, including previous stints as a Regional Nurse and Director of Assisted Living at two senior living companies. Additionally, he has experience in hospitality management.
 
He earned an associate’s degree in science and graduated as valedictorian of his nursing school class in 2015. He is a licensed vocational nurse, holds an RCFE license and served five years in the United States Navy, where he was a Combat Medic. 
 
Jimmy was inspired to pursue a career serving seniors after his own mother was diagnosed with dementia. He is dedicated to helping others who live with dementia live their best lives in the care of CountryHouse.

Danielle Peck brings more than a decade of experience in senior living to her role at CountryHouse. As a Senior Living Consultant, she works closely with families considering CountryHouse for their loved ones.

Danielle is passionate about working with and advocating for seniors. She holds a Bachelor’s degree in Health Sciences – Gerontology and holds an RCFE License. 

Giam Alviedo is CountryHouse Folsom’s Director of Nursing. Giam earned a Bachelor of Science in Nursing in his native country, the Philippines, and completed EMT certification through Sacramento State University. He also holds his RCFE license. 

His first stint in assisted living was as a caregiver. He later became a Med Tech, and then was promoted to Resident Services Coordinator, where he managed the staffing schedule and assisted the community’s Director of Nursing. He later worked as a Nurse Supervisor and a Nurse Director.

Giam is thankful for the opportunity to serve those at CountryHouse, and works to treat all in his care like family. 

 

Todd Criner is CountryHouse in Folsom’s Maintenance Supervisor. He joined the team in March 2022. 

A handyman with a heart for seniors, Todd enjoys making a difference in the lives of those who reside in our community by ensuring their home is beautiful and well-maintained. He is a proud Folsom native.

Rostam Hooshidary was named Director of Dining for CountryHouse in October 2022. Prior to that, he was the Director of Culinary Services for another senior living community for eight years. Before that, he was a Chef at Red Hawk Casino for eight years, and was an Executive Chef at El Macero CountryHouse Club for twelve years.  

He enjoys cooking a variety of cuisines, including Mediterranean, Italian, Persian, Mexican, Spanish and American. He is also experienced in hospitality management. 

A native of Iran, Rostam enjoys giving back to seniors and loves it when seniors and their families enjoy the dining experience that he creates. 

Thea McDonald is our community’s Life Enrichment Coach, and joined the team in June 2022. 

In her role, she contributes to the physical, social, spiritual and intellectual wellness of those at CountryHouse by planning and leading a wide array of activities meant to stimulate the mind, body, and soul. 

In her spare time, Thea enjoys baking, spending time with her partner, Michael, and loves everything Disney.

 

Katherine Martinez has been a valued member of team CountryHouse since November 2021. In her role, she works diligently to keep the community running smoothly, and serves as a right hand to the Executive Director. 

Prior to her current position, she spent a decade in restaurant and hospitality management in several different cities including Los Angeles, San Jose and Monterey. 

Katherine was driven to pursue a career in Senior Living in order to impact others and thrive in her professional life. She’s found that working with seniors brings great joy and a feeling of fulfillment.

Leticia Higares, LVN, is Agemark’s Regional Nurse. She works directly under Agemark’s Corporate Nurse, and provides guidance to wellness directors in Agemark’s seven communities in the state of California. Prior to her current role, Leticia was the Executive Director at Astoria Senior Living and Memory Care in Oakdale, CA, an Agemark Senior Living community. Prior to joining the Agemark family, she served as a health and wellness director for six years. She is a licensed vocational nurse and spent a decade as an HR manager. She has a degree in Business Administration. Leticia and her husband, David, have four adult children, three dogs and make their home in Morada, CA. She was born and raised in the Bay Area.
