CountryHouse in Elkhorn opened in January 2021. Our gorgeous community is designed purposefully for those with dementia. At capacity, we are home to 40 residents who are cared for by a team specifically trained to help those who live with the challenges brought forth by memory loss. A sister community, in nearby Millard, is just six miles away.

Our Leaders

The heart of CountryHouse really shines through in our people. Our team members understand the difference between caring and giving care, and they’re committed to infusing our philosophy of treating people like family into everything they do.

Jeania Bartlett joined team CountryHouse in July 2022. In her role as the community’s administrative assistant, she works to ensure the community runs as smoothly as possible, serving as a right hand to our Executive Director.

Her first foray into senior living came in 1996, where a job as an administrative assistant at a community in Idaho quickly transitioned into roles in marketing and advertising in the same community. After two years in that role, she made a career shift and worked in insurance sales, as a loan officer, a patient care coordinator, a flight attendant and as a supervisor and welder on submarines. Directly prior to joining CountryHouse, she worked at Boys Town. 

Jeania and her husband are natives of the pacific northwest. They have six grown children and many grandchildren. Jeania was born in Coeur d’Alene, Idaho, and loves the outdoors, particularly waterskiing and spending time around a campfire. She and her family believe in giving back to others, and do so through their ministry, “Pure Game,” which works to assist youth struggling with addiction. 

“I love working with our residents and showing them love and compassion. During a busy workday, breaking away from work to share a meal with them, or taking a short walk hand-in-hand is the greatest gift.”

Kimberly Duckworth is the Life Enrichment Coach at CountryHouse in Elkhorn. In her role, she works to ensure the physical, spiritual, intellectual and social health of all residents in the community. 

Kimberly brings a wealth of experience with seniors to CountryHouse. She holds a certificate in gerontology and phlebotomy and completed the activity director program at Iowa Western. Additionally, she spent time as a CNA, a CMA and is a certified dementia practitioner. 

She was prompted to join the CountryHouse team after visiting a friend, a resident of CountryHouse that she’d previously worked with at another community. The welcoming atmosphere, caring staff and home-like feel of our community made an impression. The rest is history! 

Kimberly and her husband have four grown children and four grandchildren. In her spare time, Kimberly enjoys camping, biking, kayaking and spending time outside. She also enjoys reading and is teaching herself the piano. 

“I’ve loved being around seniors since I was a little girl. Having the opportunity to care for them and bring joy to their lives is something I cherish.”

Ralph Juliano, III, brings more than 45 years of experience in food and nutrition to CountryHouse. 

Knowing he could make a difference in the lives of seniors, Ralph joined the CountryHouse team in July 2021 and was promoted to Dining Manager just one year later. He takes great pride in contributing to the happiness of residents through his kitchen expertise. He enjoys socializing with them, learning about their preferences and creating special meals upon request. He sees his work as a great privilege and desires to create meaningful days for residents by making meal time enjoyable and delicious.

He and his wife, Caroline, have been married for more than twenty years. They have two grown sons, Joe and Jack. Together, the family enjoys fishing, hunting, traveling and spending time with their three dogs: Charlie, Lola and Sadie.

“My favorite part of this job is seeing residents happy and excited to come to meals. I love my job and making a difference in the lives of residents.”
