Base Rates Plus Levels of Care: 

For residents with Level 4 assessments and above, this plan allows you to choose your room based on availability and your specific preferences. Rates for Levels of Care start at no cost for Level 1 and go up from there.

Fact #3: You may have additional costs you will need to cover.
